WHY BACTERIOPHAGES FOR BONE INFLAMMATION?

Symptoms and treatment for osteomyelitis or bone inflammation (osteitis)

In bone inflammation, the bacterial infection is the main issue. Most affected individuals feel weak and tired and suffer from general malaise and fever. After a few days, the first joint and limb pain occurs; external signs of inflammation are not yet visible at first. Only after several days do affected areas swell. A noticeable warmth can be felt from the outside.

Staphylococcus aureus and coagulase-negative staphylococci are found as the cause of chronic bone inflammation in around 75% of cases. With decreasing frequency and depending on predisposition, streptococci, Gram-negative pathogens (enterobacteria, pseudomonads) and anaerobes can be detected; mycobacteria and fungi are rare.

Phage therapy for bone inflammation

In Eastern Europe, phage therapy is used to treat bone inflammation. In particular, affected individuals who already have an intolerance to antibiotics benefit from this therapy. However, antibiotics and bacteriophages are also used in combination.

The best-selling phage cocktail contains phages that lyse or kill the following bacteria:

Streptococcus pyogenes, Staphylococcus aureus, Escherichia coli, Pseudomonas aeruginosa, Proteus vulgaris, Proteus mirabilis

As mentioned above, bone inflammation is predominantly caused by Staphylococcus aureus. For this reason, this phage cocktail is used for this infection.

Treatment with bacteriophages is usually started immediately. Since bacteriophages have no side effects, this procedure is harmless.

The goal of phage therapy is to bring the phages to the location where bacteria are present.

Use of bacteriophages for osteomyelitis

Children aged 1 to 3 years take a dose of 2x 5 ml daily, children aged 3–8 years take a dose of 2x 8 ml daily, and children aged 8 and over and adults take a dose of 2x 10 ml daily orally.

The phage cocktail is taken orally twice daily, 15 minutes after a meal.

In addition, either omeprazole 15 min before taking the phage cocktail is taken, or a glass of water (250 ml) in which 1/2 teaspoon sodium bicarbonate (baking soda) has been dissolved is drunk, in each case about 15 min before taking the phage cocktail (or according to the manufacturer’s instructions for the baking soda).

This reduces the stomach’s pH level in order to increase the survival rate of the phages on their way to the urinary bladder. The duration of use is approx. 14 days.

During treatment, an alkaline diet is followed and plenty of water is drunk (approx. 2 L/day).

If oral use is not successful, irrigation of the bone or joint with the phage cocktail is often performed in hospital.

Treating chronic osteomyelitis and osteitis is one of the greatest challenges in modern surgery, as pathogens such as Staphylococcus aureus, Staphylococcus epidermidis and Pseudomonas aeruginosa form deep biofilms in bone tissue or on osteosynthesis material (plates/screws). In current scientific assessments, bacteriophages stand out due to their ability to enzymatically penetrate these biofilms—an advantage over many antibiotics, which often do not reach the focus of infection in avascular bone at a therapeutic dose.

The Eliava Institute in Georgia has documented clinical successes for decades in saving limbs by instilling phage cocktails directly into the bone canal. In Europe, initiatives such as the PhagoBurn pilot project have significantly advanced regulatory standards for GMP-compliant phage preparations (Good Manufacturing Practice). Current Belgian collaborations and studies at Charité Berlin underscore the potential of personalised phage therapies as an adjunct to surgical debridement. The evidence suggests that the combination of radical eradication of pathogens and local phage application can significantly reduce the recurrence rate in chronic bone infections.

Is phage therapy for osteomyelitis approved in Germany? A general marketing authorisation for ready-made phage medicinal products is currently still lacking in Germany. However, its use is legally permitted as an individual treatment attempt (Declaration of Helsinki) or via a magistral formulation (individual prescription), provided that conventional antibiotic therapy has been unsuccessful.

When does phage therapy start to work for bone inflammation? Initial clinical effects, such as a reduction in local inflammatory markers, can often be observed within 48 to 72 hours after application. However, complete restoration of bone tissue is a process that can take several weeks, depending on the severity of the osteitis.

Can phages be combined with antibiotics for bone inflammation? Yes, combining phages and antibiotics is usually synergistic. Phages weaken the bacterial protective matrix in the bone biofilm, allowing antibiotics to reach the pathogens more effectively again. In research, this is often referred to as “phage-antibiotic synergy” (PAS).

Why is a phagogram mandatory before treating osteitis? Because phages are extremely specific to certain bacterial strains, a phagogram must be performed in the laboratory beforehand to check whether the selected phages can actually kill (lyse) the patient’s individual bacterial isolate. Without this matching, treatment success is unlikely.

Healthier

"Fast shipping. Well packaged. Then you have to read up a bit. I used the bacteriophages to treat my chronic, massive sinus infection, which had been treated with antibiotics on and off for years.
